Client overview
We worked on TeleMD, a care management and telemedicine platform used by clinician-owned clinics for chronic care, principal care and remote patient monitoring. Our work was on the monitoring side: live telemetry dashboards, threshold alerts and the nurse triage workflow.
The problem
Readings, triage and time lived in different places: Clinics running care management programs need device readings, the nurse triage step and the monthly care team time in one record, so nothing is missed and every minute of care is counted.
What we worked on
Threshold alerts and outreach
Readings outside a patient's range raise an alert so the team can reach out.
Nurse triage workflow
A clear path from alert to review to action, with clinical shift tracking.
Part of a wider platform
TeleMD also covers EHR and telehealth visits, care management time tracking, AI clinical scribe and intake, staffing tools, state-aware routing and native patient apps. We worked on the monitoring layer, not the whole platform.
